Job Summary
Provide in-home speech-language pathology services to patients under CenterWell Home Health. Deliver evaluation and treatment of speech, voice, language, hearing, and swallowing disorders, administer and interpret diagnostic tests, develop and submit evaluation and discharge documents, participate in care coordination and discharge planning, and educate patients, families, and healthcare staff. Maintain documentation, comply with caregiver training, and report to the Clinical Manager. Requires NC SLP license and CPR; valid driver’s license and transportation; PRN schedule. Home health setting with compassionate, patient-centered care.
Required Qualifications
- Current and unrestricted Speech Language Pathologist license in the state of North Carolina
- A valid driver‘s license, auto insurance, and reliable transportation
- Current CPR certification
