Job Summary
Home Health RN (PRN) providing skilled nursing care on a geographically assigned caseload, coordinating care from admission through discharge, utilizing physician orders to develop and implement individualized home health plans of care, supervising home health aides, and participating in on-call rotation with 5–7 patient visits per day. Responsibilities include conducting assessments (OASIS), planning and implementing nursing interventions, collaborating with physicians and the health team, documenting care, educating patients and families, ensuring compliance with state and federal Home Health regulations, and maintaining professional nursing standards. License as an Ohio RN, CPR, and valid driver are required; two years of clinical nursing experience with home health preferred.
Required Qualifications
- Graduate of an accredited nursing program
- Baccalaureate degree in nursing or related field preferred
- Current RN license in Ohio
- CPR certification
- valid driver with automobile insurance
- excellent written and verbal communication skills
- two years clinical nursing experience with home health experience preferred
