Job Summary
Home Health PRN RN provides skilled professional nursing for a geographically assigned caseload, developing and implementing a Home Health plan of care in accordance with physician orders and state/federal regulations. Responsibilities include conducting initial assessments (OASIS), planning and coordinating patient care, supervising home health aides, collaborating with physicians and the health team, documenting accurately, and participating in on-call, weekend, and holiday rotations. Requires leadership in planning and implementing care, teaching patients and families, and ensuring quality outcomes with 5–7 visits per day.
Required Qualifications
- Graduate of an accredited nursing program
- Baccalaureate degree in nursing or related field (preferred)
- Current license as a Registered Nurse in the State of Ohio
- CPR certified
- Must be a licensed driver with automobile insurance
- Minimum of two (2) years clinical nursing experience in general nursing with home health experience preferred
