Job Summary
Home Health PRN Registered Nurse (RN) provides one-on-one, in-home nursing care across an assigned territory. Responsibilities include completing skilled nursing assessments, developing and updating individualized care plans, conducting admissions, routine visits, recertifications, and discharge visits; evaluating clinical and psychosocial needs; coordinating care with physicians, caregivers, and the interdisciplinary team; monitoring patient progress; educating patients and families on disease management and self-care; maintaining timely clinical documentation; and helping prevent avoidable hospitalizations through proactive interventions. Requires MA RN license, at least 1 year clinical nursing experience, willingness to travel, valid driver’s license, and experience with HomeCare HomeBase (preferred).
Required Qualifications
- Massachusetts RN license (unrestricted)
- Minimum 1 year nursing experience in a clinical setting
- Associates or Bachelors Degree in Nursing
- Valid driver's license, reliable transportation, current auto insurance
- Willingness to travel within assigned territory
Desired Qualifications
- Current, unrestricted Massachusetts Registered Nurse (RN) license
- Minimum of one year of nursing experience in a clinical care setting
- Associate or Bachelor's Degree in Nursing
- Valid driver's license, reliable transportation, and current auto insurance
- Willingness to travel throughout the assigned territory
- Hospice experience preferred
- Experience with HomeCare HomeBase (HCHB) preferred
