Home Health PRN D Speech Therapist

Deliver speech-language pathology services in home health settings, performing evaluations, determining SLP diagnoses, developing plans of care, and providing therapy to restore/maintain speech, language, motor, and swallowing abilities. Conduct timely evaluations and reassessments (OASIS) to determine functioning levels, update plans of care with the physician and interdisciplinary team, provide therapeutic treatments and equipment assessments, document clinical notes, plan for discharge, and participate in interdisciplinary meetings. Build collaborative relationships with the multi-disciplinary team, coordinate with Home Health Aides, participate in training and support of team members, volunteers, and students, and ensure adherence to applicable guidelines and regulatory standards. Must hold a Master's in Speech Language Pathology, SLP licensure (ASHA CCC-SLP), and VDHP licensure upon hire, with CPR/BLS certification within 30 days and a valid driver's license for travel.
