Job Summary

Provide comprehensive 1:1 client care in client homes for conditions including epilepsy, cerebral palsy, and quadriplegia, administering medications and treatments as ordered. Evaluate client status, interpret symptoms, and report significant changes to the Case Manager immediately while maintaining accurate clinical records. Adapt nursing measures to changing needs, educate families, and collaborate with the interdisciplinary team to ensure continuity of care. Participate in scheduled conferences and the after-hours on-call process to resolve staffing issues. Maintain a safe environment, adhere to infection control policies, and uphold confidentiality. Requires a Minnesota RN license, two years of experience, and ability to drive or use public transportation in all weather conditions.

Required Qualifications

  • Currently licensed as an R.N. in the state of Minnesota
  • The license must not have been revoked, suspended, and without limitations or restrictions
  • Have at least two (2) years' experience in a health care setting
  • Demonstrates ability to make appropriate nursing judgments
  • Possess good interpersonal communication and teaching skills
  • Good organizational skills and the ability to work independently
  • Excellent written and oral communication skills
  • Proof of negative mantoux or documentation of negative chest x-ray
  • A valid driver's license and proof of car insurance when using a personal vehicle for company business
  • Have U.S. Citizenship or evidence of valid Alien Work Permit
  • Discloses any conviction and criminal history records pertaining to any crime related to the provision of health services
  • Pass initial and ongoing background studies and screenings including but not limited to those of the Minnesota Department of Health and the Federal Office of the Inspector General's List of Excluded Individuals and Entities
  • Must be able to drive or use public transportation in all types of weather
  • Must be adaptable to a variety of environments and community settings
  • Must be able to lift 100 pounds of force occasionally, and/or in excess of 50 pounds of force frequently, and/or in excess of 20 pounds of force constantly
  • Must be available for weekend shifts
  • Must be available for Saturday shifts
  • Must be available for 4pm-9pm Monday-Friday
