Job Summary
Plan, provide, and evaluate nursing care for clients with conditions like epilepsy, cerebral palsy, and quadriplegia in their homes. Administer medications and treatments as ordered, interpret symptoms, and institute remedial measures within legal limits. Document observations, assist clients with medical appointments, and maintain accurate clinical records. Recognize and report significant changes in client condition to the Case Manager immediately. Exercise professional judgment in adapting nursing measures and accepting assignments. Participate in the after-hours on-call process to resolve staffing issues. Maintain a safe environment, identify potential abuse or neglect, and communicate effectively with the interdisciplinary team. Adapt to diverse family environments and drive to multiple client stops daily.
Required Qualifications
- Currently licensed as an L.P.N. in the state of Minnesota
- The license must not have been revoked, suspended, and without limitations or restrictions
- Have at least one (1) year experience in a health care setting
- Demonstrates ability to make appropriate nursing judgements
- Strong interpersonal communication and teaching skills
- Strong organizational skills and the ability to work independently
- Excellent written and oral communication skills
- Proof of negative mantoux or documentation of negative chest x-ray
- A valid driver's license and proof of car insurance when using a personal vehicle for company business
- Have U.S. Citizenship or evidence of valid Alien Work Permit
- Disclose any conviction and criminal history records pertaining to any crime related to the provision of health services
- Pass initial and ongoing background studies and screenings including but not limited to those of the Minnesota Department of Health and the Federal Office of the Inspector General's List of Excluded Individuals and Entities
- Access to a dependable vehicle or public transportation in order to travel to multiple company business stops a day
Desired Qualifications
- Acute hospital or home care experience
- Current CPR certification
