Job Summary
Home Health PTA to implement individualized plans of care for geriatric patients across a California territory. Responsibilities include executing plans of care developed by the supervising Physical Therapist, performing therapeutic interventions focused on functional strengthening, gait training, balance, and fall risk reduction, documenting patient progress, coordinating with the supervising PT, and managing your own caseload with flexible scheduling. The role uses TherapySync EMR for charting, referrals, authorizations, and billing support, enabling independent, patient-centered care in a diverse home-health setting.
Required Qualifications
- Valid California Physical Therapy Assistant license
- CPR or BLS certification
- Driver's license and auto insurance
- Physical + TB test (within the last year)
- Professional liability and business license (if applicable)
