Job Summary
Home Health PTA—execute care plans and deliver patient-centered care across a broad Los Angeles-area territory. Implement individualized plans of care developed by the supervising Physical Therapist; perform functional strengthening, gait training, balance, and fall-risk reduction to help patients reach mobility goals. Document patient responses and communicate with the supervising PT to ensure a coordinated approach. Manage your own caseload with a flexible schedule and visit cadence. Work within TherapySync EMR for compliant charting and referrals/authorizations handling, with support from a team for billing and scheduling.
Required Qualifications
- Valid California Physical Therapy Assistant license
- CPR or BLS certification
- Driver's license and auto insurance
- Physical + TB test (within the last year)
- Professional liability and business license (if applicable)
