Job Summary
PRN Home Health Physical Therapy Assistant in La Grange area providing physical therapy services per physician plan of care in patients’ homes. Responsibilities include assessing therapeutic status, evaluating mobility and ROM, directing therapy, instructing patients and families, prioritizing therapy needs, reporting responses to treatment, and participating in case conferences. May train others, assist with prosthetic devices, coordinate referrals for additional home health services, prepare clinical progress summaries, participate in discharge planning, provide home health aide supervision as needed, and contribute to in-service education. Flexible schedule with administrative support and mobile geographic coverage; work location is on the road with visits to patients' homes.
Required Qualifications
- Graduate of an accredited physical therapy assistant program with either an Associate's degree in science or applied science that is approved by American Physical Therapy Association, Inc.
- Current certification or license as required by state law.
- Two years of appropriate experience as a physical therapy assistant. Community/home health experience is preferred.
- Demonstrates good verbal and written communication and organization skills.
- Possesses and maintains current CPR certification.
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
- Must possess Independent general liability workman’s comp coverage/cert.
