Job Summary
Deliver patient-focused physical therapy services as a Home Health Physical Therapy Assistant (PTA) under the supervision of a Physical Therapist. Responsibilities include adhering to treatment plans, assisting in preparing clinical and progress notes, educating patients and caregivers per the plan of care, maintaining effective teamwork and relationships with colleagues and the community, upholding service standards for exceptional customer care, and communicating with patients by phone or in person. Requires graduation from a PTA curriculum accredited by APTA, active PTA license, and at least two years of PTA clinical experience; valid driver's license and appropriate auto insurance are required. Facilities are part of the CommuniCare Family of Companies serving Ohio, Indiana, Maryland, Virginia, West Virginia and Pennsylvania with a focus on high-quality, person-centered care.
Required Qualifications
- Graduate of a Physical Therapy Assistant curriculum accredited by the American Physical Therapy Association (APTA)
- Active PTA license
- At least two (2) years of clinical experience as a Physical Therapy Assistant (PTA)
- Valid Driver’s License and automobile insured in accordance with state/organization requirements
