Job Summary
Home Health Physical Therapy Assistant provides direct physical therapy services to patients in their residences under the direction of a physical therapist and the Home Health leadership team. Responsibilities include administering therapy plans, guiding active and passive exercises, gait and functional training, prosthetic training, and use of modalities (heat, cold, water, light, electricity, ultrasound, massage) to relieve pain and restore function. The role also involves documenting in the patient’s clinical record, instructing patients and families in care and use of DME, observing patient responses to treatment, and communicating changes to the physical therapist and care team. Requires PTA program completion, a state license, and the ability to use an electronic medical record; requires reliable transportation. Desired qualifications include 2 years of PTA hospital or home care experience. Office location: Jackson, MI with service area Jackson and surrounding. Required Qualifications
- Successful completion of a physical therapy assistant program at a school of physical therapy approved by the American Physical Therapy Association
- Physical Therapy Assistant License recognized in the state in which practicing
- Demonstrates knowledge and skills necessary to provide care to and communicate with the adult and geriatric populations
- Demonstrates knowledge of the principles of growth and development over the life span
- Able to assess data reflecting the client's status and interpret the appropriate information needed to identify each client's requirements relative to their age-specific needs
- Demonstrates computer skills sufficient to use the electronic medical record effectively
- Reliable transportation and valid automobile insurance

- 2 years experience working as a Physical Therapy Assistant-hospital and/or home care experience preferred
