Job Summary
Provide skilled physical therapy services under the direction of a licensed physical therapist in patients' homes within Davidson County, focusing on pain relief and restoration of function per the physician's plan of care; educate patients and caregivers on rehabilitation techniques and safe use of assistive devices; collaborate with an interdisciplinary team; perform documentation and discharge planning, maintain confidentiality, and ensure timely progress notes and credential submissions; requires PTA education and an active state license plus CPR.
Required Qualifications
- Gradation from a Physical Therapy Assistant education program from an accredited university/program
- Current, unrestricted state license as a Physical Therapy Assistant in the state of practice
- Valid CPR
