Job Summary
Provides skilled physical therapy services in home health under the supervision of a physical therapist, including treatment per physician plan of care, patient/caregiver education on rehabilitation and use of assistive devices, discharge planning and documentation, and collaboration with interdisciplinary team to optimize patient well-being. Requires PTA education, active state license, CPR, and adherence to health-related requirements; located in Antioch, IL with full-time schedule Monday–Friday and field-based patient visits.
Required Qualifications
- Gradation from a Physical Therapist Assistant education program from an accredited university/program
- Current, unrestricted state license as a Physical Therapy Assistant in the state of practice
- Valid CPR
- Vaccinations as required by jurisdictions (Medicare/Medicaid)
- Compliance with health-related requirements as appropriate
