Job Summary
Provide skilled in-home physical therapy services as a licensed PT or PTA on a per diem/PRN basis in O‘ahu, HI. Perform evaluations, reassessments, and fall risk assessments; develop and implement individualized treatment plans to improve mobility, balance, strength, and safety; educate patients and caregivers on home exercise programs and fall prevention; document progress and communicate with an interdisciplinary care team; maintain Medicare home health documentation compliance. Flexible schedule with per-visit compensation; require Hawaii license, valid driver’s license, and reliable transportation; experience in home health preferred and ability to work independently in the field.
Required Qualifications
- Current Hawaii license as a Physical Therapist (PT) or Physical Therapist Assistant (PTA)
- Minimum one year of clinical experience (home health experience preferred)
- Strong documentation, communication, and time-management skills
- Ability to work independently in the field
- Valid driver’s license and reliable transportation
- Passion for serving seniors, kupuna, and local families
