Home Health Physical Therapist with Generous PTO

Conduct ongoing clinical assessments including OASIS evaluations to identify patient needs and determine appropriate home care levels. Develop and evaluate plans of care in partnership with patients, caregivers, and physicians while providing education on transfer techniques and equipment use. Perform treatment for neuromuscular, neurological, and orthopedic disorders and supervise physical therapy assistants and home health aides as appropriate. Document clinical notes, phone orders, and records timely while applying infection control standards. Accept clinical assignments consistent with education and competence to oversee patient care activities.
