Job Summary
Assess patients' needs in homes throughout Osceola, Volusia, Seminole, Orange, and Brevard counties to develop effective treatment plans focused on improving mobility, strength, balance, and functional independence. Perform in-home physical therapy evaluations, teach caregivers mobility techniques and safe device usage, and maintain accurate clinical documentation while communicating progress to the care team and physician. Collaborate with nursing, therapy, and administrative teams to ensure timely completion of authorized visits and comprehensive care. Requires current, unrestricted Florida physical therapist license; full-time position with grouped visits to minimize drive time.
Required Qualifications
- Current, unrestricted Florida physical therapist license
- Ability to cover multiple counties (our service area includes Osceola, Volusia, Seminole, and Orange Brevard)
- Must have the ability to pass a background check and drug screening test
Desired Qualifications
- At least 1 year of physical therapy experience
- Ideally in an outpatient, rehabilitation, home health, or geriatric care environment
- Experience performing home health evaluations and completing related documentation
