Job Summary
Assess, interpret, plan, implement, and evaluate patients with appropriate skill based on age and diagnosis. Administer medications, perform wound care, and provide individualized patient care for adults and geriatrics. Educate patients and families on health management and self-care techniques while collaborating with the healthcare team. Admit patients when physical therapy is a primary skilled need and refer to other professionals as ordered. Communicate and document information in accordance with agency policies, participate in program development, and maintain a daily commitment to agency values. Requires a valid North Carolina driver's license, operational vehicle, and ability to lift over 50lbs and transfer patients over 100lbs. Works 40-hour weeks Monday through Friday with variable scheduling including weekends and holidays as needed.
Required Qualifications
- Graduate of Physical Therapy program from an accredited university
- Licensure in the state in which providing therapy (NC/SC)
- CPR certification
- Minimum 1 year experience
- Knowledge of home health and physical therapy procedures
- Ability to manipulate patient care and evaluation equipment, including modalities and assistive devices
- Ability to properly transfer and guard patients
- Ability to apply other manual therapy techniques
- Ability to deal with the public, co-workers, patients, families, and physicians in a professional and positive manner
- Significant locomotion
- Frequent lifting greater than 50lbs
- Transferring patients greater than 100lbs
- Ability to see and communicate clearly in order to perform basic clinical techniques
- Ability to apply physical therapy clinical principles and use professional reasoning to develop appropriate treatment programs
- Ability to prioritize duties, learn new skills and techniques in patient care
- Ability to learn and use computer technology and supportive services
- Valid North Carolina driver's license
- Operational vehicle
