Home Health Physical Therapist

Monitor patient progress, adjust interventions as necessary, and participate in discharge planning for smooth transitions. Maintain accurate documentation using appropriate medical terminology. This role involves working on the road in various settings including hospitals, clinics, and outpatient facilities. Day shift availability is required.
