Job Summary
Deliver physician-ordered physical therapy services in patients' homes and supervise PTAs/HHAs as needed. Conduct initial assessments, revise plans of care every 30 days, educate patients and caregivers on exercises and home care, and prepare timely clinical documentation including OASIS. Collaborate with the care team on treatment adjustments, participate in quality initiatives and patient care conferences, and manage a home-based caseload with independent work style in a telehealth-enabled/home health setting. Requires NM PT licensure, accredited PT program graduate, strong assessment/care planning skills, and a valid driver’s license.
Required Qualifications
- Current licensure as a Physical Therapist in New Mexico
- Graduate of an accredited Physical Therapy program
- Strong assessment and care planning skills
- Ability to work independently and manage a home-based caseload
- Excellent communication with patients, families, and team members
- Valid driver’s license and reliable transportation
- Prior home health experience preferred, but new grads are welcome!
