Assess, teach, and deliver physical therapy interventions directed by the Physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels while administering tests to determine goals and outcomes. Provide written evaluations and modify treatment plans based on client performance, ensuring timely documentation to prevent denials. Collaborate with interdisciplinary team members, physicians, and caregivers to optimize patient well-being and coordinate discharge planning with home exercise instructions. Supervise personnel as applicable and complete OASIS assessments where permitted. Maintain effective communication with staff and families while adhering to all licensure regulations and patient care standards.
