Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Deliver physical therapy interventions directed by the physician while collaborating with interdisciplinary team members to optimize patient well-being. Provide written evaluations, modify goals and treatment plans as needed, and manage discharge planning including educating caretakers on home exercise programs. Complete OASIS assessments, maintain accurate documentation to prevent denials, and supervise personnel such as PTAs and HHAs. Serve as a clinical resource consultant while adhering to all licensure regulations and care standards.
