Job Summary
Assess patient mobility, coordination, and strength to establish functional goals and outcomes. Deliver physical therapy interventions directed by the physician while maintaining the plan of care through timely documentation and goal modification. Provide written evaluations defining physical therapy needs and expected outcomes, including discharge planning with patient and caretaker education on home exercise plans. Collaborate with interdisciplinary team members, physicians, and nurses to optimize patient well-being and coordinate services. Supervise personnel such as PTAs and HHAs where applicable. Complete OASIS assessments where permitted by state regulations.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Demonstrated proficiency in clinical assessments, documentation and compliance with policies and procedures
- Required vaccinations and testing
