Job Summary
Deliver home health physical therapy services to patients in Tomah, WI, including initial and ongoing assessments of mobility, strength, coordination, and functional level; develop and document a plan of care with goals and expected outcomes; provide therapy per physician orders; discharge planning and patient/family education on home exercise programs; coordinate services with the healthcare team; supervise PTA/HHA as applicable; complete OASIS assessments where allowed by state practice regulations; maintain documentation in accordance with policies to support payer reviews; participate in interdisciplinary coordination to ensure quality patient care.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR
- Preferred: Experience in Home Health Therapy and completion of OASIS documentation
