Job Summary
Provide written evaluations and plans of care defining physical therapy needs, goals, progress, and expected outcomes. Deliver physical therapy services in compliance with prescribed physician orders while maintaining discharge care plans through timely documentation of client performance. Collaborate with physicians, nurses, and families to coordinate services and educate patients on home exercise plans. Ensure documentation meets payer requirements to avoid technical or clinical denials and adhere to all licensure regulations and patient care standards. Requires a current unrestricted state license, OASIS documentation experience, and a degree from an accredited program. Salaried full-time position with Monday through Friday flexible visit times and a $20,000 sign-on bonus.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Experience in Home Health Therapy
- Completion of OASIS documentation
- Required vaccinations
- Testing
