Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Deliver physical therapy services complying with prescribed physician orders while administering, evaluating, and interpreting tests to determine functional goals and outcomes. Provide written evaluations and plans of care, modify treatment as needed, and create discharge care plans with patient and caretaker education. Coordinate services with physicians, nurses, and other healthcare team members, and supervise personnel such as PTAs and HHAs. Complete OASIS assessments where permitted and document effectively to prevent technical or clinical denials. Maintain communication with staff and families while adhering to all policies, licensure regulations, and state laws.
