Job Summary
Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Deliver physical therapy services complying with prescribed physician orders while administering, evaluating, and interpreting tests to determine functional goals and outcomes. Provide written evaluations and plans of care defining needs, progress, and expected outcomes, ensuring documentation results in no technical or clinical denials. Maintain discharge plans by collaborating with the healthcare team and educating patients and caretakers on home exercise routines. Supervise personnel including PTAs and HHAs where applicable, and complete OASIS assessments per state regulations. Work Monday through Friday with flexible visit times in the La Crosse/Onalaska territory.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR
- Compliance with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing
Desired Qualifications
- Experience in Home Health Therapy
- Completion of OASIS documentation
