Job Summary
Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current function. Deliver physical therapy services complying with physician orders while administering, evaluating, and interpreting tests to determine functional goals and outcomes. Provide written evaluations and plans of care, modify treatment as needed, and collaborate with the healthcare team to coordinate discharge planning and educate caretakers on home exercise plans. Maintain effective communication with staff and families, supervise personnel as applicable, and complete OASIS assessments where permitted. Work Monday through Friday with flexible visit times in the La Crosse/Onalaska territory.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR
Desired Qualifications
- Experience in Home Health Therapy
- completion of OASIS documentation
