Job Summary
Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Deliver physical therapy services complying with physician orders while administering, evaluating, and interpreting tests to determine functional goals and outcomes. Provide written evaluations and plans of care, maintain documentation to prevent denials, and collaborate with the healthcare team, physicians, and caregivers for discharge planning and home exercise education. Supervise personnel including PTAs and HHAs where applicable. Complete OASIS assessments where permitted by state regulations. Monday through Friday with flexible visit times in the La Crosse/Onalaska territory.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR
- Required vaccinations and testing
Desired Qualifications
- Experience in Home Health Therapy
- completion of OASIS documentation
