Job Summary
Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current function. Deliver physical therapy services complying with physician orders while administering, evaluating, and interpreting tests to determine functional goals and outcomes. Provide written evaluations defining needs, progress, and expected outcomes, then modify treatment plans and discharge care plans as appropriate. Collaborate with physicians, nurses, and caretakers to coordinate services and educate patients on home exercise plans. Document client performance, complete OASIS assessments, and maintain effective communication with the healthcare team and family. Supervise personnel including PTAs and HHAs where applicable.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR
Desired Qualifications
- Experience in Home Health Therapy
- completion of OASIS documentation
