Conduct initial and ongoing assessments of patient mobility, coordination, strength, and functional levels. Deliver physical therapy services per physician orders while administering, evaluating, and interpreting tests to establish functional goals and outcomes. Create written plans of care detailing needs, progress, and expected results, then modify treatment and discharge plans as needed. Complete OASIS assessments where permitted and ensure documentation accuracy to prevent clinical denials. Collaborate with physicians, nurses, and family members on discharge education and home exercise plans. Supervise physical therapy assistants and home health aides as applicable.
