Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels for patients recovering from illness or injury in their homes. Deliver physical therapy services per physician orders, document client performance, and modify treatment plans to achieve functional goals. Provide written evaluations, discharge care plans, and educate caretakers on home exercise routines while collaborating with physicians, nurses, and other healthcare team members. Complete OASIS assessments where applicable and supervise personnel such as PTAs and HHAs. Maintain effective communication with staff and families to ensure accurate documentation and clinical outcomes.
