Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Deliver physical therapy services complying with prescribed physician orders while administering, evaluating, and interpreting tests to determine functional goals and outcomes. Maintain plans of care through timely documentation, modifying treatment as needed, and determining discharge care plans. Collaborate with physicians, nurses, and caretakers to educate patients on home exercise plans and supervise other personnel such as PTAs and HHAs. Complete OASIS assessments where permitted and ensure documentation meets payer standards to avoid denials.
