Home Health Physical Therapist Salaried Full Time 20K Sign on Bonus

Provide written evaluations and plans of care defining physical therapy needs, goals, progress, and expected outcomes. Deliver services complying with prescribed physician orders while maintaining plans through timely documentation of client performance and modifying goals or treatment as needed. Collaborate with the healthcare team, physicians, and caretakers for discharge planning and education on home exercise plans. Ensure effective communication between staff, team members, and families while serving as a clinical resource consultant. Adhere to all policies, procedures, licensure regulations, and state laws to prevent technical or clinical denials. Requires a Physical Therapy degree, current unrestricted state license, home health therapy experience, and OASIS documentation skills.
