Job Summary
Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Deliver physical therapy services per physician orders while administering evaluations to define goals and outcomes. Maintain plans of care through timely documentation, modifying treatments as needed, and determining discharge care plans. Collaborate with physicians, nurses, and families to coordinate services and educate patients on home exercise plans. Supervise PTAs and HHAs where applicable and complete OASIS assessments per state regulations. Work Monday through Friday with flexible visit times in the La Crosse/Onalaska territory.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR
Desired Qualifications
- Experience in Home Health Therapy
- completion of OASIS documentation
