Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Deliver physical therapy services per physician orders while administering, evaluating, and interpreting tests to set functional goals. Provide written evaluations and plans of care, modify treatment as needed, and coordinate discharge planning with physicians, nurses, and caregivers. Supervise PTAs and HHAs where applicable and complete OASIS assessments. Maintain effective communication with the healthcare team and family, ensuring documentation meets payer standards to avoid denials. Work Monday through Friday with flexible visit times in the La Crosse/Onalaska territory.
