Job Summary
Conduct initial and ongoing mobility, coordination, and strength assessments to evaluate patient function. Deliver physical therapy services per physician orders while administering, evaluating, and interpreting tests to set functional goals. Maintain discharge plans by collaborating with the healthcare team, educating caretakers on home exercises, and modifying treatment as needed. Complete OASIS assessments and ensure documentation complies with payer policies to prevent technical denials. Supervise PTAs and HHAs where applicable. Monday through Friday flexible visit times in La Crosse/Onalaska with a $20,000 sign-on bonus.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR

- Experience in Home Health Therapy
- completion of OASIS documentation
