Home Health Physical Therapist Salaried Full Time 20K Sign on Bonus

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Deliver physical therapy services complying with physician orders while administering evaluations to define goals and outcomes. Maintain care plans through timely documentation, modifying treatment as needed, and determining discharge plans with patient education. Coordinate services with physicians, nurses, and families, supervising PTAs and HHAs where applicable. Complete OASIS assessments and ensure documentation accuracy to prevent clinical denials. Work Monday through Friday with flexible visit times in the La Crosse/Onalaska area.
