Job Summary
Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Deliver physical therapy services complying with prescribed physician orders while administering, evaluating, and interpreting tests to determine functional goals and outcomes. Provide written evaluations and plans of care, maintaining documentation to prevent technical or clinical denials. Collaborate with physicians, nurses, and caretakers for discharge planning and education on home exercise plans. Supervise personnel including PTAs and HHAs where applicable, and complete OASIS assessments where permitted by regulation.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR

- Experience in Home Health Therapy
- completion of OASIS documentation
