Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels for patients recuperating at home. Deliver physical therapy services compliant with physician orders while administering, evaluating, and interpreting tests to determine functional goals and outcomes. Provide written evaluations and plans of care, modify treatment as needed, and manage discharge planning by educating caretakers on home exercise plans. Maintain effective communication with the healthcare team and family, supervise personnel such as PTAs and HHAs, and complete OASIS assessments where permitted. Document client performance accurately to ensure no technical or clinical denials, collaborate with physicians and nurses, and adhere to all licensure regulations and policies.
