Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels for patients recuperating at home. Deliver physical therapy services complying with physician orders while administering, evaluating, and interpreting tests to define functional goals and outcomes. Maintain care plans through timely documentation, modifying goals as needed, and creating discharge plans that educate caretakers on home exercise routines. Coordinate services with physicians, nurses, and families, and supervise PTAs and HHAs where applicable.
