Provide written evaluations and plans of care defining physical therapy needs, goals, progress, and expected outcomes. Deliver physical therapy services to comply with prescribed physician orders while maintaining plans through timely documentation of client performance and modifying treatment as needed. Collaborate with the healthcare team, physicians, and caretakers to coordinate services and educate patients on home exercise plans for discharge. Document effectively to avoid technical or clinical denials, maintain communication with staff and families, and serve as a clinical resource consultant. Work Monday through Friday in Southern Jackson and Northern La Crosse counties with flexible visit times.
