Job Summary
Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Deliver physical therapy services complying with prescribed physician orders while administering, evaluating, and interpreting tests to determine functional goals and outcomes. Provide written evaluations and plans of care defining needs, progress, and expected results, maintaining documentation to prevent denials. Collaborate with physicians, nurses, and families on discharge planning and home exercise programs. Supervise personnel including PTAs and HHAs where applicable, and complete OASIS assessments as permitted. Work Monday through Friday with flexible visit times in the La Crosse/Onalaska territory.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR
Desired Qualifications
- Experience in Home Health Therapy
- completion of OASIS documentation
