Conduct physical therapy services to comply with prescribed physician orders while providing written evaluations defining patient needs, goals, and expected outcomes. Maintain care plans by documenting client performance, modifying treatment as needed, and determining discharge care plans through collaboration with physicians, nurses, and caretakers. Ensure documentation meets payer standards to avoid technical or clinical denials while serving as a clinical resource for staff. Requires a Physical Therapy degree, current state license, and experience with OASIS documentation. Schedule includes Monday through Friday with flexible visit times.
