Job Summary
Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Deliver physical therapy services compliant with physician orders while administering, evaluating, and interpreting tests to determine functional goals and outcomes. Provide written evaluations and plans of care defining needs, progress, and expected outcomes, including timely documentation and discharge planning with caretakers. Collaborate with physicians, nurses, and other healthcare team members, supervise PTAs and HHAs, and complete OASIS assessments where permitted. Maintain effective communication with staff and families while adhering to all licensure regulations and payer documentation standards. Monday through Friday with flexible visit times in the La Crosse/Onalaska area.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR
Desired Qualifications
- Experience in Home Health Therapy
- completion of OASIS documentation
