Job Summary
Deliver skilled, patient-centered care in a home health setting by performing comprehensive patient evaluations, functional mobility assessments, and developing individualized therapy plans to improve strength, balance, and independence. Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies while working independently within an assigned territory. Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation. Travel to patient homes within Grayling and surrounding areas, spending approximately 20% of your day driving. Requires an active Physical Therapist license, one year of experience, and a current driver's license. Eligible for up to $20,000 in total bonus potential, including a first-year success bonus and sign-on bonus.
Required Qualifications
- Active Physical Therapist, PT license in the state you work
- One year of experience as a Physical Therapist
- Clinical expertise to deliver quality patient care
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
- Successful completion of a drug screen
- Legally authorized to work in the United States
