Job Summary
Deliver skilled, patient-centered care in a home health setting by performing comprehensive patient evaluations, functional mobility assessments, and developing individualized therapy plans to improve strength, balance, and independence. Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies while thinking critically to solve problems in real time. Work independently within an assigned territory, ensuring thorough, compliant EMR documentation and collaborating with an interdisciplinary team. Requires an active Physical Therapist license, one year of experience, and a current driver's license to travel to patient homes, spending approximately 20% of your day driving. This full-time role offers $20,000 in total bonus potential, including a $10,000 first-year success bonus and a $10,000 sign-on bonus, alongside medical, dental, and vision benefits.
Required Qualifications
- Active Physical Therapist, PT license in the state you work
- One year of experience as a Physical Therapist
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
- Must be in good standing & meet performance expectations
- Must be legally authorized to work in the United States
