Job Summary
PRN Home Health Physical Therapist provides physical therapy services per the physician's plan of care, including evaluating therapeutic status, determining rehab needs, directing therapy, instructing patients and families, and coordinating discharge planning. Responsibilities involve training patients in prosthetic device use when needed, preparing clinical progress summaries, supervising PT assistants, and offering in-service education to nursing staff. Requires a PT degree, state licensure, CPR, and a valid driver’s license with vehicle; minimum of two years of relevant experience; on-the-road home health work in Seneca, IL.
Required Qualifications
- Degree from a baccalaureate or masters program in physical therapy
- Licensed to practice as physical therapist in the state (where applicable)
- Two years of appropriate experience as a physical therapist
- Current CPR certification
