Job Summary
PRN Home Health Physical Therapist in Indian Head Park, IL area providing physical therapy services per physician’s plan of care, including patient assessment, mobility and functional status evaluation, patient and caregiver instruction, and collaboration with home health team. Responsibilities include supervising PT assistants, assisting with discharge planning, providing PT consultation to families, and ensuring progress summaries and referrals as needed; requires a PT degree, state license, CPR, driving with insured vehicle, and at least two years of relevant experience.
Required Qualifications
- Possesses a degree from a baccalaureate or masters program in physical therapy
- Licensed to practice as physical therapist within the state
- Two years of appropriate experience as a physical therapist
- Demonstrates good verbal and written communication and organization skills
- Possesses and maintains current CPR certification
- Must be a licensed driver with an automobile that is insured in accordance with state requirements
- Must possess Independent general liability workman’s comp coverage/cert
Desired Qualifications
- Bachelor's or Master's degree in Physical Therapy
- Licensed to practice as a physical therapist in the state
- CPR certification
- Valid driver's license and insured vehicle
- Two years of relevant physical therapy experience
- Strong verbal and written communication and organizational skills
