Job Summary
Physical Therapist PRN delivering direct patient care in home-health settings, evaluating functional needs, designing and implementing care plans, and guiding staff to achieve patient goals. Responsible for accurate documentation, ongoing assessment of outcomes, and collaboration with other specialists. Travel up to 50% within the branch/office coverage area; requires lifting and transferring patients and equipment, and working in a high-demand, potentially irregular-hour environment. Qualified candidates will have an APTA-approved PT program and an unrestricted PT license, with opportunity to contribute to community-based rehabilitation and patient-centered care.
Required Qualifications
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
- Current, unrestricted PT license valid for the state of work
- Minimum one (1) year of experience in a healthcare facility
- Able and willing to travel within branch/office coverage area
- Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws
